Job Title: Case Manager (Contract‑to‑Hire)

Location: Long Beach, CA 90804

Pay: $23–$24/hour

Position Overview

LHH is seeking a Case Manager for a client in Long Beach. The Case Manager provides direct support and advocacy for clients, helping them access essential services while building long‑term life skills. This role is highly on‑site and fast‑paced, requiring strong boundaries, resilience, and the ability to manage frequent client interactions. This is a contract‑to‑hire opportunity.

Key Responsibilities

• Conduct client intake, assessments, and individualized service plans
• Provide ongoing case management and monitor client progress
• Connect clients to housing, employment, healthcare, and community resources
• Maintain accurate documentation in compliance with program requirements
• Collaborate with internal teams and external partners
• Participate in team meetings and trainings
• Uphold confidentiality and organizational policies

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field (or equivalent experience)
• Prior case management experience preferred, especially in housing or social services
• Strong organizational, communication, and interpersonal skills
• Ability to manage a high‑interaction, on‑site environment
• Proficiency in Microsoft Office and case management systems
• Bilingual English/Spanish a plus
• Valid driver’s license with no more than 2 points
• Willingness to drive a company vehicle or travel once converted to permanent
• Comfortable working with a direct, no‑nonsense hiring manager
